IMPORTANT: When renting our large equipment please ask your liability insurance company for a Certificate of Liability Insurance (COI) that contains the following.*

• The name and address of the insured customer- (you.)

• Dutch Rentalz, 243 Market Ave SW, Hartville, Ohio 44632 must be listed as the certificate holder.

• Dutch Rentalz must be listed as additional insured and loss payee.

• Proof of general liability of $1,000,000 minimum.

• Proof of physical damage coverage on the rented equipment with limits greater than the value of the equipment rented.

• Policy number for contractors equipment coverage and effective / expiration dates of coverage.

Rented Equipment coverage must be equal to or greater than the value of the equipment rented.
